This re-label request form is for Barenbrug USA seed only. It is important to fill out all ALL fields on this form. Seed that is more than two years old can not be re-labeled.

As you are putting your grass seed away for the season, please complete this form so we can send you new labels for next season. We will send these out prior to the season. Seed not re-labeled prior to the season can not be offered for sale.
